Does something need to be fixed, maintained, or adjusted? Complete a request for services.

Did harm or damage occur? Report as incident.

Could harm or damage occur if nothing changes? Report as hazard.

Request for service (maintenance/repair)

Something that needs to be fixed, maintained, or adjusted to keep facilities, equipment, or infrastructure operating as intended.

Examples:

  • A tap that is leaking
  • A door that is not closing fully
  • A light that is not working
  • Damaged fixtures or fittings requiring repair

Incident

Something has already happened that caused harm, damage, or loss.

It involves an actual event or outcome, not just a possibility.

Examples:

  • A worker slips and injures their ankle.
  • A vehicle collision occurs on site.

Hazard

Something that could cause harm or damage but hasn’t yet.

It’s a potential risk or unsafe condition that needs attention before it leads to an incident.

Examples:

  • A spill on the floor that hasn’t caused anyone to fall yet.
  • Frayed electrical cords near a walkway.
